To pay by credit card, please call us at 866-464-8893 to give us your payment information.

For credit card payments we will need: the credit card number, expiration date, and the security code on the back of the card (on Visa, Mastercard, and Discovery, it is the 3 digit code on the back of the card, and on American Express it is the 4 digit code on the front of the card.

If you are faxing your order, manually write your payment information on the form and fax the order form to us toll free at 866-464-8893.

If you are paying by check or money order, then you can mail your order and payment to the address on this form. If you would like to pay by bank wire, please contact us to request our bank information.
